NEW PATIENTS WELCOME
We will make sure you are comfortable and relaxed first. The dentist would like to know you as a person before any dental work is discussed. A tour around the clinic will be given as well.
A new patient form will be offered to fill-out (you can download forms below). You can fill them at home and bring with you on your appointment if you wish to save some time.
Information that will be helpful:
⦁ Dental x-rays less than one year old or contact information of the dental office where they were taken.
⦁ List of the medication if you take any.
⦁ Contact information of your medical doctor or other specialists if applies.
⦁ Dental insurance card if you would like us to send claims for you.
⦁ Sometimes new set of radiographs may be necessary to offer you the most detailed and comprehensive exam.
⦁ We will address your chief complain first. Enough time will be given to fix the dental problem if you have any, at the same day or another appointment will be scheduled.
⦁ We will accept ALL KINDS OF DENTAL BENEFITS and will file dental claim after treatment directly to your insurance on your behalf.
⦁ NO INSURANCE – we have some DISCOUNTS available for patients with low income. Please call for details.
